Many are using Massachusetts, the state that’s already been through its own health reform wringer, as a bellwether for national reform. President of Business Benefits Insurance in Andover, Mass., Jim Edholm expects little impact from PPACA on his business. “I don’t see a whole lot changing because we’re already at such skimpy levels of commissions.”
To wit, Massachusetts recently passed legislation requiring the state’s MLR in 2011 across all size business not to exceed 88% – 90% by 2012. Says Edholm: “That’s hardly going to cause [state carriers] to break a sweat,” as approximately 95% of the business is nonprofit HMOs. “Already I think all four of the major carriers – BlueCross BlueShield of Massachusetts, Harvard Pilgrim, Tufts Health Plan and Fallon Community Health Plan – and probably the fifth, Neighborhood Health Plan, [are] in that range. So they’re already pretty efficient.”
As for other states, “in most of the rest of the states they have to be prepared for lower commissions,” says Edholm. “It’s going to happen and there’s nothing you can do about it.”
